Accessing Patient Records

Patients are entitled to access to their personal RFC medical records under the Data Protection Act 1998 and Access to Health Records (NI) Order 1993.

To obtain a full copy of RFC fertility records you need to make their request  in writing and send to :

As fertility medical records for couples attending the RFC are held in one chart, both partners need to sign the letter requesting a copy.  

An administration charge of £50 is made by the Belfast Trust for a full copy of records.  If requesting only test results, a one off charge of £10 is made, regardless of how many results are requested.  Payment must be sent with the written request (Cheques should be made payable to Belfast Health & Social Care Trust)

For patient confidentiality reasons full copies of records must be collected in person, usually by the patient.
Where the patient authorises another person to collect on their behalf, a letter of authorisation must be provided in advance by the patient naming the person who will be collecting the records.
Photographic ID is required to be presented to RFC staff on collection of the copy of the record.

Patients who have undergone treatment at the RFC may request a treatment summary which includes semen analysis and female blood test prior to treatment.
